Stroke Association: Hertfordshire Stroke Recovery Service
If you or someone you know has had a stroke (and is over the age of 18), we’re here with help and support from day one and for as long as you need us. Our Stroke Recovery Service offers practical information, guidance and support after a stroke.
A Stroke Association Support Coordinator will contact you to organise a phone call and/or home visit. They’ll work with you to identify your priorities and what’s important to you so that we can best support you to rebuild your life after stroke. We’ll also connect you to other people affected by stroke who understand what you are going through.
Who can use this service?
This service is for people (over the age of 18) who have had a stroke (including TIAs/mini-strokes) living in Hertfordshire, their carers and family.
How are services delivered?
You can be referred to the service by a Health Care Professional or community organisation. You will be contacted by telephone to discuss your support needs. Or you can self-refer using the contact details provided.
