Reach Out: Hertfordshire

Reach Out provides support for those who have recently been discharged from hospital or at risk of admission. Reach Out can assist people by linking them up with a volunteer in their neighbourhood who can offer companionship and practical support when they get home. This service is free. Please note that in order to meet our support criteria, the client must be over 50 and recently discharged from hospital, at risk of admission and living with frailty.
